• Identity Data: Name, email, phone number, password.
• Service Data: The specific "Service Bundle" or appointment type you select. Note: This reflects your interest in a health service.
• Payment Data: Transaction details (amount, date). We process payments via Stripe and do not store full credit card numbers.
• Provider Data: NPI, license details, banking info (for payouts).
• Usage Data: IP address, device type, approximate location (City/Zip).

3. How We Use Information

• To Provide the Service: Booking appointments, sending confirmations, processing payments.
• Safety & Security: Verifying identity (2FA), preventing fraud.
• Communication: Sending appointment reminders or updates.
• Legal Compliance: Tax reporting, complying with subpoenas.

4. Sharing of Information

• With Providers: We share your Name, Contact Info, and Selected Service Bundle with the Provider you choose.
• With Processors: We share data with vendors who help us run the business (e.g., Stripe for payments, Cloud Providers for hosting).
• No Sale of Data: We do not sell personal information or consumer health data to third parties.

5. Consumer Health Data Notice (WA & NV Residents)

Because you use our platform to book health services, we process "Consumer Health Data" (CHD) as defined by Washington and Nevada laws.

• Sources: Collected directly from you.
• Purposes: To facilitate the booking you requested.
• Sharing: Shared with the Provider you explicitly selected.
• Rights: You have the right to access, delete, and withdraw consent. Email support@costplus.care.

6. HIPAA Disclaimer

• Our Role: Generally, we are a technology vendor.
• Business Associate: If we contract with a HIPAA "Covered Entity" (Provider), we act as a "Business Associate."
• User Warning: Do not submit sensitive medical charts/diagnosis notes into our general "contact" forms.

7. Tracking & Analytics

We use cookies and analytics to improve our Platform.

• Privacy Guard: We configure these tools to avoid collecting sensitive health data for advertising purposes.
• Controls: You may disable cookies in your browser settings.

8. International Transfers

Our servers are located in the United States. If you access the Platform from outside the US, you acknowledge that your data will be transferred to and processed in the US.
